Taking one policy and paying for two

I took a policy with real people on 2011for my family meaning myself, my husband and my three kids. Instead in 2012 there was another policy deduction to my account and I report the matter to my consultant (Ntomboxolo Maka) and she promised to check what was actually happening till to date. I try to trace the second policy and to my suprise I was paying a policy for people that I do not know. I want all my premiums to be paid back to me and I would also like to know who signed contract document for me. I am not working I took my policy from the disability grant that I've got as I am partially blind. I do not have any relatives that appear on policy number 11981438 and I really do not took that policy. Please help me!!!
