Charged for a Card - Not Received

I was notified to collect a card, that I did not want.
I did not collect the card, and two months later (in November 2013), I was still charged R65.00 for the annual card fee.
I went to the Branch to query this transaction, and was told it was for the card - upon stating that I do not have the card, they tried to find the card to give to me - which I did not want, nor did they have the card (it was destroyed)
The Branch manager stated that she would investigate the problem and notify me of the outcome.
This was more than three weeks ago.
I now received my bank statements which indicate that I did not get the credit for the card.
I find it unacceptable to be charged for a card I do not posses, nor want, nor did I get it when I went to the Branch.
Besides the fact that this card when I previously had one, gave me more problems than before - that is why I did not accept the newly issued one.
How can I be charged for something that I did not get?
I require this matter resolved, as I am in my 30 days to query transactions (which I did already three weeks ago) or my statements will be considered correct. I need this resolved.
