Refusing te refund after ATM Faulty

On the 23rd October 2013 in the morning I went to Standard Bank ATM, Sasol Garage, cnr Ben Swart Street and HF Verwoerd Drive in Gezina, I tried to withdraw R400 from the ATM and the ATM processed the transaction without paying out the money as well as not supplying me with a slip. When I tried again I only received a slip saying that there was insufficient funds. I was late for my doctor's appointment and had to borrow money to pay said doctor. I am with Oneplan medical and have a debit card to pay for medical services. I went to the doctor and after reported the problem to Standard Bank Hatfield Branch where I was given ref: 55596355 and told to wait 15 days and the money would be paid out. On 12 November 2013 I phoned Standard bank Customer Services on 0860 101 341 and was informed by Fareza that the inquiry had been closed! Standard bank had not advised me of this even though they had my cell number registered on the query. I contacted Oneplan and Robert referred me to Tutuka on 011 803 3115 where Lungi took down my details. Yesterday afternoon I was advised that I needed to complete a dispute form. Today I am being told I have to send an Affidavit signed at the police station.
