Not able to read a form correctly

Death claim submitted in Feb this year. It has now been almost 4 months. The last email supposedly sent to me on the 25 April requested outstanding requirements which are clearly on the form and I confirmed this with a call centre agent. We a loved one passes away surely the sanlam should have more qualified people working on death claims as this in itself is a difficult time that loved one will be going through. I need this claim to completed so that I can pay my outstanding hospital bills.!!!
