Delay paying out death claim

I have submitted all required information for the payout of a death claim at end of March and followed up with a telephonic call and was told that all the documentation was submitted to complete the processing of the claim.
Week after week the same request is send to forward further information which was already submitted and therefore no pay out has been made to date. On Friday 3 May 2013 another e-mail was sent requesting information that was already sent. Further delays to payout a claim.
Please assist ASAP.
