Continuous bad service after many promises

My husband passed away in September 2013. Our cellphone contract was on his name. I have been trying since end of September to transfer the contract to my name. I filled in all the forms at the Menlyn branch who sent it to headoffice. In December 2013 after the third visit to the branch Chester promised to personally handle my account and sort it out as it was still not transferred. I never heard from him again! After another visit I was informed that I need to supply new original bank statements which was allready done September 2013. On Wednesday 22 Jan 2014, I personally delivered the statements at the Menlyn branch for attention of Jaques who phoned me about it. On 27 Jan I phoned as I received an sms from headoffice about the account, stating that I will be penalized because there is no debit order for the payment yet.. I phoned Jaques at Menlyn who had no knowledge of my bank statements nor had David to whom I handed it. I still do not know whether the contract has been transferred and according to Jaques the bank statements are missing. I am extremely upset about the way this matter was handled and will definitely not renew my contract with Autopage when it expires
