Still processing a fraundulent claim?!

I called in and reported activity on my account - being upgraded without my knowledge.
I have been given a reference number but now I have received an sms saying that the new phone has been shipped and invoiced. Is hold not put onto this before you proceed.
I also called in to 0821959 and when I keyed in my cell number I was told that the number I have dialled does not exist (I tried twice)
what format must the affidavit take. is there a sample?
