Account Migration without consent leads to triple

In October 2013 my account my migrated without my consent and my bill went from R1000-R1900 pm to R7500pm to date,
I have made several attempts to try and rectify the account even requesting a Manager contact me which failed, (please see attached)
My last call to my service provider to conduct a audit as I could use such high usage on the account, (check account history), thereafter I received a respond via sms stating my account was being audited and this will take 14 working days and a consultant would contact me, regards (Maynard Abrahams), prior to this in November 2013 I logged a call, without any feedback a consultant sent me an sms stating my account was up to date with payment regards (Delray Jacobs),
Today I received another Vodacom invoice stating my bill for Dec 2013 was R7900 which will be debited on the 25th Jan 2014, I cannot afford to pay these bills, this is 80% of my salary monthly, yet Vodacom have been debiting me without any resolution,
Who Migrated my Account in Oct 2013, as I did not sign a document and send back to Vodacom
Who removed the Soft lock of my account Permanently,
Why was it not reinstated after me requesting
my acc is still debited now in a
