Account terminated, still paying for itemised bill

I terminated my account in September 2012, as the contract was complete and I could get better deals elsewhere.

Recently I noticed that I am still being charged for itemised billing - I get a monthly account with nothing but a R25 charge for itemised billing - :(

I want my money back and I want the debit order to stop.

I also want to be refunded for the bank charges this ongoing error has incurred.
