Insurance claim nightmare- stupid loan phone

The courier company was supposed to bring a sim adaptor as I make them aware that my phone is a samsung note 2 and I'm using a micro sim but they gave me a loan form that is not micro-sim compatible. I refused to take the phone. They promised we they will bring the phone again with the sim adaptor on Monday. I had to call again on Tuesday. They gave me the loan phone and I didn't receive an sms to notify me that they have received my phone from the courier company and it has been booked with the technicians. I tried calling for 2 weeks without any luck. I got through and the consultant hung up on me. I tried everyday holding on up to an hour without an answer. I tried calling mtn business and asked if they can be able to assist with this matter. One told me she is transferring my call to make sure I go through and I was put back to the same line I have been trying to call. I called again and was promised a team leader will call me. I managed to get through after 3 weeks and they told me my phone was returned back as I declined the quote, Where was the phone returned to? How did I decline something I didn't receive. Today the IVR says the delay is due to hand set still to be booked
