Please send me a statement/invoice???

I have been an ADT Subscriber since June 2012 and have been charged for ADT Armed Response on a monthly basis without ever receiving a monthly statement/invoice for the service via email or post???
What's worse is the debit order amounts fluctuate as ADT feels necessary however I'm unable to query this as I do not have a statement to question.
In November 2012 I was charged and additional R210 for an annual license fee which I had already paid in June when the contract was signed, why???
Will I ever receive a statement from ADT for the Armed Response service and why was I charged an Annual license fee in November 2012 when I had already paid this amount in June 2012?
I have tried logging this query with their call centre in November and was assisted by Thabo Kekana who eventually sent me my Account Profile. Thabo then said the license fee would be reversed which has happened as yet. I have responded to Thabo's email asking for assistance but I have received no response.
Please advise???
