Signed up for Jet Club without my consent

I called the service centre twice on 17 October regarding a club magazine that I never signed up for and a Club together fee that is charged on my account. I apparently signed up May 2013 but this was the first time I got the magazine. This is the closest I ever came to receiving a statement from Edgars after requesting it to be emailed to me on about six occasions. I was told to go to Jet stores in Cresta to query how I was signed up, I went to the store and they could not find any proof that I signed up yet the fee is charged on my account every month without fail. Ive called in thrice now to ask them when these fees will be reversed and about a dozen times for me to receive my statements via email. They are good at signing you up for things you did not request and can post these magazines but they cannot send you a statement whether post or email or resolve your query when you call the call centre.
My request is simple, I need the club fees on my account to be reversed since from May 2013 to date and I need my statements to be mailed to me monthly. This far no-one has been able to assist me with this.
