Listed me on ITC for a mistake by accounts departm

Even after cancellation of my subscription with Multichoice last year they kept charging my credit card for several months. After a lot of tiring and stressful conversation with them I managed to get back my money. Then after few months I again started receiving statement with arrears due. When I enquired why are they sending me this then they had no explanation. I again went back to call the agent and spent hours with them only to understand that they have refunded me for an extra month. I eventually paid that one month arrear again to them on 9th October 2012.
Now I am hearing from my bank that I have arrears of R904 against Multichoice. That means they have put me on ITC for the mistake they did.
Now after speaking to an agent last week she wrote email to legal department for me to send the written communication to take me off their list as I have no dues against them but till date I haven't received a response.
I want my name removed and that too with written feedback. Please call me and sort this out ASAP
