Liberty Messes Up Yet Again

On 19 March 2013 I had gone for my second procedure of my root canal. The previous procedure had already been covered. When checking my profile today, I see Liberty has rejected my account, claiming I required pre-authorization.
I then called to the call centre, & Sello Mlatji informed me that I required pre-auth from the DRC. I had no idea about this, because Liberty did not inform me, let alone send me a member guide for 2013. I find it strange that the previous visits to the same dentist, for the first part to this procedure, did not require authorization, & was covered.

Now I'm supposed to attempt getting pre-auth (for a procedure which occurred almost 1 month ago) & hope they will cover, otherwise I will have to cover the bill, yet again, because of Liberty's short-fall on giving me proper service!

This is the second time Liberty Medical has messed up on my profile, in the space of 2 months.
When on earth will the standard they claim to have, be met?

This is ridiculous! It should not be up to me to fish for answers...
