Refusing to refund me Breach of Contract!

I am on the most expensive medical plan on Bonitas Medical Aid called BonComprehensive.. I was told this is the elite and will cover all my medical requirements.
I was explained the following:
You have a certain amount of savings money allocated to you for the year. When the savings run out you reach SELF PAYMENT GAP. This means you have to pay a certain amount cash out of your own pocket..
Then you reach THRESHOLD. In THRESHOLD ALL ACCOUNTS ARE 80 % PAID BY YOU and I will be liable for 20 %.

This is not the case.

There was an outstanding dentist bill total l R4099.75 from September 2012 to November 2012. At this point I'm THRESHOLD.
I was told am personally liable for it and action will be taken if this bill it not paid soon.

I was in my THRESHOLD and Bonitas STILL REFUSED TO PAY the bill. So now I paid this bill out of my own pocket. I require my refund as what Bonitas advertised is once you in THRESHOHLD THEY WILL PAY 80 % OF THE BILLS. I want to be refunded this money and it is already FEBRUARY 2013.

This is total breach of contract. You are NOT delivering what you promise and what I signed and am paying for
