Lying on not receiving request

I requested a change in my medical aid option due to being unemployed. I was requested to submit a form. Which I duly submitted on the day the request was made (10 December 2013). I receive a email on 31 December 2013 stating that they did not receive my change from and therefore cannot change me to BonCap. How did they know I wanted to change to BonCap if they did not receive my form. They are lying and am planning on charging them with perjury with the medical aid ombudsman.
