No response to reinstatement of membership

I requested cancellation of my Bonitas Standard option on or around 15th Sept 2013.
Nobody at Bonitas tried to assist by saying although my Income has changed they can provide me with a lower option to save costs.
I e-mailed Bonitas on 29th Sept asking them to send Benefits brochure and change of option form, got a response and was inforomed someone will contact me. I am yet to receive a call!!
I phoned Tracy on 30 Sept 2013 and she promised to send the info and advise whom I need to contact re reinstatement. I also received no response.
I phoned Ntabaseng today and she said she would send me the info and change of option form.
I have also not received this!!
I have been without medicl aid since 01 Oct 2013 and find the service disgusting yet again!!!
