Why penalties and premiums loaded?

I was planning to add my mother to my medical aid but was told that she would not have benefits for 3 months and her premium would be loaded by 25%. Why? My mother worked for and belonged to a medical aid for her entire life. Currently she is a member of Fedhealth - why in 's creation would her premium be loaded and her benefits withheld for 3 months?
