Motivational letter to be submitted

I am experiencing problems with my uterus, and the Gynae gave me 2 options - the Novasure or a hysterectomy. My history on Bestmed will show I have already had the Mirena as well - that did not work - that was taken out. Not going to put all detail here.
You have NO idea what pain I am in every month, and now you need a motivational letter from the Gynae, and IF you decide his letter is not motivational enough to your criteria, then Bestmed won't pay for the procedure.
Just funny, that Bestmed takes our debit order every month, but the day you need to get an authorization to go to hospital, there is a problem. And this is not the 1st time I need to get a motivational letter.
I understand there are some chancers, but we as honest people are always "punished" because of them.
The procedure is much less costly than a hysterectomy... so will Bestmed rather pay much more for a hysterectomy???
My procedure is on 30 July 2013.
