Pathetic service/feedback regarding medical claims

I am writing this out of sheer desperation and disgust. I have been trying for months now to be reimbursed for medical expenses which is covered under this insurance. I receive no feedback other than a generic message saying the case has been closed, when in fact it has not. Just this past week alone: 13 phone calls, one lasting 32 minutes! Countless e-mails, which has bounced back from their system since last week Monday. Fax Wed 8 Jan 2013, no response, fax 11 Jan 2013, no response. Eventually I was given an alternate fax number, tried 4 times, each time it says, no answer. Faxed again on 15 Jan 2013. Everyone I talk to says they will escalate it to a consultant, yet nobody calls you back. My issue: apparently Oneplan will not reimburse you for claims older than 4 months. My issue: a claim from Feb 2012 NOT PAID even though I followed instructions from their representative on the process to follow. Documents were submitted which they now conveniently cannot find. Second gripe: pathology acc they will not reimburse because its older than 4 months, first I knew of the acc was in Dec when I received a final demand letter. E-mailed them Nov/Dec/Jan - no answer. I am at wits end
