3 months, Temporary disability, Shocking delay

My company filed a report for temporary disability on my behalf more than 2 months ago. Momentum took 1 and a half months to then stipulate that I have to see their preferred practitioner.
After a very difficult and uncomfortable session with the doctor, it has been another month passed and the preferred doctor has not done the report - despite me calling daily.
Not having this temporary disability matter solved, I am under even more stress with my condition.

Please someone help!!!
