Charges on card never received

Absa phoned and offered me a credit card. I accepted, and had to just collect papers to complete at the East Rand Mall branch. They saw fault with address, paper work etc the entire time. I had to drive up and down various times. Till today they wont give me the card. BUT every month they send me a statement, that even credit card number on. I have phoned, gone in to the branch various times; they dont even allow me to close the account. Why must I pay for a bank card I never used or collected from them in the first place!!! I currently owe them R958.68... I have been trying to get this sorted / cancelled for about two years now. I am getting bad credit on my name because of Absa for something, that the application was never even completed!
