ATM transaction not reversed

My wife tried to withdraw money at an instore FNB ATM on on the 6th of September. The machine went offline and indicated that it had dispensed the cash. She called an instore assistant which said that she had to contact FNB. We completed the dispute forms on Monday the 9th and got a reference. Ref : 20130909_112813797 which was sent via email.
Since then no response from them and seemingly no one able to help me at the premier banking call centre.
Being a premier client I am shocked at the poor response thus far- this should be a matter that FNB should correct instantly and automatically without any serious hang-ups.
