Death Claim for Police no SL 090159877 not paid

Good day
In 2004 August my Dad passed away, we reported that to Momentum, they send us claim forms to fill in and send back, we didnt have the origional copy of the contract but an affidavit was made with a death certificate and all the certificates and ids of the benefitiaries. No money was received. When tried to call them last year 2012 they still requsting same documents we've sent 9 year ago.
