Same "Mess up" two times in a row

I have previously complained regarding my mix-up when I added on an additional phone to my contract in Jan 2013. My new contract was put onto someone else's name. Yet again, the same thing has happened 2 months down the line and again I get so off the the disgusting level of service that I return the phone and the manager has nothing to say and gladly takes the phone back. I am a current customer and spend around R2500-R3000 per month and to get this kind of service is absolutely disgraceful to the Brand. I must say that as soon as my contract is up I will no longer stay with Vodacom and would rather go with a service provider who actually gives a about their customers and business as clearly Vodacom does not value both. I try to complain telephonically and just get put on hold and then I get told that someone will call me back and nobody ever does. This contract wasnt taken for the fun of it but for business use and now everyone has just wasted my time. The last time I complained nothing really happened and I hope that someone actually takes this seriously
