Non Cancellation of Phone Line after 2 months

I am moving to a new house and went into the direct shop to request them to cancel my current phone and install a new one at my new address. I was told to call the call center to perform this task. The call center then referred me to the Telkom website [URL Removed] and asked me to complete this form.
I have now completed the form 3 times since the end of November 2012 and still have not been contacted by Telkom, and I am still receiving invoices for the line in my old flat. I am assuming the new tenants are using the phone as Telkom have not cancelled it as I asked for on numerous occasions.
