Worst service ever and keep deducting money!

I have a mobile contract with them which I have not used for over a year. They kept calling to upgrade me and I kept telling them "no thank you". Then I called to cancel the contract and was told to complete the forms and return via email which I did on 24 October 2013. Bear in mind that my 24 months have by then expired already. I made numerous calls to them and nobody could tell me what is going on and why they keep deducting the premium every month. This morning received another invoice from them and once again they deducted the premium off my bank account. Upon calling them, I was told that the call centre can't see the invoice details as it is now prepaid, how if I get an invoice? Then accounts tells me they cancelled w.e.f. 1 Feb... Now I need to follow up again in 7 working days... And I keep paying the penalty for their incompetence! And nobody available to authorize and help with a refund... They are the worst ever service provider. This is definitely not service, they are from their clients.
