Multiple breaches of contract. Highly dissatisfied

I have an AnyTime200 contract and have been experiencing problems from day 1. I never received my monthly provision on the 21st of October and the 21st of November 2013, thus my call and data usage was charged at the out-of-bundle rate. When I received an outrageous bill, I proceeded to query the matter. 3 months and many calls to customer care later and I am still having issues. The consultants were extremely unhelpful and one of them, as a "quick fix", loaded extra data bundles onto my account which I never requested yet still had to pay for. I am also still waiting for an itemised billing report as well as a URL report (which I paid for) to arrive (3 months and still nothing).
I feel that MTN are in serious breach of contract in numerous ways and I refuse to pay for things I am not receiving as well as pay for things that I did not request.
To top it all off, I don't even have proper cellphone signal where I live. Calls consistently drop when I am at home and I can never use 3G. I am lucky if I even have a single bar of EDGE reception.

I am not prepared to deal with this terrible service any longer.
