Insurance not captured by consultant

I applied for a contract phone for my son on 28/12/2013 and was assisted by Saajidah. She discussed and quoted insurance for R77which I agreed upon and accepted with my son and daughter as witnesses. I received an sms later that week and was assisted by Funda on 31/12/2013, who at first seemed reluctant because Saajidah was not there, but went ahead and captured the deal on the system, had me sign documents, and handed me the phone. On 03/01/2014, the phone fell out of my son's pocket and the screen cracked.

On 07/01/2014 I took the phone to the store to be sent for a quote before a claim could be submitted to the insurance. On receiving the quote and wanting to proceed with the claim, I was told that I did not have insurance. I confirmed with Saajidah that I applied for insurance, Funda refuses to take responsibility for not capturing the insurance. The manager Clinton Tobias retrieved the original application and I was not surprised that he could not find any application for insurance. Funda to my face, claiming that he discussed the insurance options with me, but on the application form, no option is ticked.
Clinton believes Funda incapable of making mistakes!
