Unable to Blacklist phone for a month

I have tried to blacklist my phone on numerous occasions, via the call center & in store. Each time I am either referred to another store or told to call the call center. I have been bounced around for a month from store to the call center with no success. I have to black list the phone for insurance purposes & I cannot get a new phone until this is resolved, as the police require an ITC number after black listing the phone & insurance requires a case number from the police.
No one in store or at the call center seems to be able to help me as every time they try blacklist it there is an error message, the same error has come up whether I go in store or call for the last month.

I am now at my wits end as I am without a phone & no one can or is willing to get to the root of the problem.
