Upgrade turned into second phone contract

I recently applied for an upgrade online through the vodacom website. It said I was due for upgrade end July. I took out a S4 phone Upgrade via their online services.
2-3 DAYS later I received my new phone S4. Without any documentation or call from anyone.
I couldn't use my current sim because I had a Nokia D600 and needed to apparently move my number to a new smart sim.
I couldn't get to any vodacom shop for a while and had an opportunity to pop into vodacom branch tygervalley today. I also enquired why my account has been so high for the past 2-3 months because its not reflecting the same amount on my online account.
When the consultant showed me my details on the system he said i have now two contracts instead of one and no upgrade took place.
How did this happened? I don't want a second contract and never wanted one i just wanted to upgrade my current contract. Can this be sorted out please because now i'm paying for two contracts I wasn't even aware of.
Now its said its my fault and that i have to pay the full amount for a second phone account contract.
Upset and wiling to close all my accounts.
