INCORRECT DEBIT: JULY 2013 - STILL NOT RESOLVED

I received my account at the end of July 2013 indicating an amount of R794.41 owing, and the said amount was debited from my bank account on the 1st of August 2013.
I immediately contacted Autopage, seeing as my account is for a 2GIG Special, in the amount of R149.00 per month and has been that way since July 2011, when I took the contract. Suddenly my contract was migrated to an open contract - WITHOUT MY REQUEST / PERMISSION!!! Till today, 27 September 2013 the amount of R645.41 (R794.41 - R149.00) has not been refunded to me despite me phoning every other week to follow up on the matter. Extremely poor service!
I have also requested a copy of my contract on 18 September 2013, was told it will take 24 working hours - and I still have not received it and it is now 27 September. I want my money refunded into my bank account - NOT CREDITED TO MY DATA ACCOUNT, and I want my copy of my contract please. Should I not receive either one of the two by 1 October 2013, I will proceed with legal action.
