Extra Insurance charges added without permission!!

I took out a contract in February 2013 for two lines 079 889 2628 and 079 889 2605 for R169 each. Now suddenly since June 2013 they are deducting R85 each for insurance for which I did not ask for or gave my permission to. And there is a R100 Billed to my account that I do not know off
Both lines has been disconnected due to the outstanding invoices which I am not willing to pay because of the insurance that adds an extra R170 per month.

I WANT THE INSURANCE CANCELLED AND REFUNDED TO MY ACCOUNT BEFORE I MAKE ANY PAYMENTS FOR MY OUTSTANDING BALANCE.
