Unknown reason for an excessive amount off my acc

I received a message from my ABSA account stating that Autopage had deducted a crazy amount off my account this month. I have just signed up for another 2 years contract with an iPhone 5 and had an insurance claim 2 months before on a Nokia N8. In July I paid the insurance excess and then this month it was almost double the amount taken off? When I immediately phoned Customer Care last night the guy told me to contact the insurance department as it seemed like an insurance claim was submitted on an iPhone 5? I have the iPhone in front of me and did NOT submit any claim for it. When I, telephonically, upgraded the guy never told me about a once off fee for the iPhone either. If this is the case I would not have taken the iPhone as I know I cannot afford a once off fee. Please explain this to me as I'm now short on cash for other accounts? It's honestly insane how this has happened without any warning whatsoever!
