Insurance claim declined

My IPhone 4S was 04/07/2013 i went to the midrand mtn branch to claim insurance, all documents where supplied and filled then today 15/072013 am informed that the insurance was declined due to non payment received in the month of June 2013, i changed my account number & visited the midrand branch and made a manual payment.
My question:
When a debit order is made i dont see any split payment in my statement it goes through as one payment, when i made manual payment of R578.00 was i supposed to make a split payment?
Why wasnt i informed that the payment/ insurance hasnt been paid?
