R4000 account on a R150/Month package

On 20 April I went to the Mtn shop in The Mall @ Reds to upgrade my 2gig data package to the uncapped light package. I was made aware by Ashleigh that the new package would only take effect at the end of the month. As it turned out it was the end of the billing month (13th of each month.), and not the last day of the month as I thought. On 15 May my connection was cut and I went to the shop the next day where I was told that I have reached my account limit of R4000!!! I never requested any limit be set on my account nor for any additional data or charges be added to my account without my consent. Jaco filled in a Query Follow up sheet and said he would log a call with Mtn SP and let me know when he had feedback for me. That evening I logged a call myself as well (Ref:100188450). When I went back to the shop to follow up on 27 May, Ashleigh took out the form from the same file he put it in 11 days earlier with absolutely nothing done on the case. He didn't even logged the call. As the end of the month approached I grew more and more nervous as I cant afford to pay R4000 for something I didn't request. What can I do to have the money returned to me?
