MTN records reflect an upgrade done in Febr 2013

I have a cell phone contract with MTN that I initially attained thru my previous employer. In June 2012 I resigned and opted to keep the contract under my personal name i.e. migrated to personal account. The migration only took place in November 2012 and last month I discovered that my account with MTN is still under Corporate Services yet I am paying this account under my personal name. I made a few phone calls with MTN to try and resolve the matter, over and above I sent Carri-Ann an e-mail enquiring about my upgrade as I was due for an upgrade last year and that never transpired as I did not get a new handset. She has never bothered to revert back to me regarding the upgrade matter. Today when I called MTN was informed that I did an upgrade in February 2013 and according to my knowledge no one from MTN issued me with a new phone or informed me of an upgrade. This is so unprofessional because I'm paying for services that I am not even getting as I was never given an opportunity for an upgrade. Now this means MTN has a record of specific phone that I do not even have in my possession and paying for that handset package that I'm not even enjoying. The reason I require an upgrade
