A debit order that I didn't authorise

I requested that my contract be cancelled, in the 23rd month as would be requested; they cut off my sim card too early. They promised to refund and never did. Now a month after, I have another debit order on a different date go off for 3 times the amount of money I ever paid! I have not signed for them to take another contract or upgrade at all!
I have alerted them, given them half a month to take action and nothing has been done! I cannot believe that a "mistake" of this illegal nature can even me made on the system.
I do not wish for the money to be deducted for a second time!
This is illegal and unacceptable.
My privacy and banking account have both been horribly violated!
