Contract Cancelled yet billed

On 02/11/2013 I applied for a contract upgrade on my 0744991784 data line. My premium was R149 and the upgrade premium was going to be R99 a month. I did not have the upgrade fee which was required instore but requested that the contract be cancelled unless the upgrade fee could be deducted together with the premium on the next debit order date as i did not have money at the time. Llewellyn confirmed that the upgrade fee would be deducted on the next debit order date.

A deduction of R149 was made on the 15th of November from my account, I did not query this as I thought the changes would take effect from 01/12/2013. From 01/12/2013 i could not access the internet using the data card only to be informed by the customer service that my contract had been cancelled on 04/11/2013.
I contacted CenturionMall store via e-mail and Llellyn confirmed that my contract had been cancelled as i had requested. I cannot dispute as i completed both the upgrade as well as the cancellation forms and left them instore. If my contract was cancelled on 02/11/2013 then why was R149 debited on 15/11/2013 & R563.01 on 01/12/2013?
I have queried instore and followed up on e-mail but still no joy.
