Declined cancellation of already expired contract

I went to the Rivonia branch on 1 October 2013 to have my contract cancelled. I took a copy of my contract with to confirm that due to it being expired for approx two years, I had a 'month to month' membership (Verify the date of expiry on your system). This means I am not liable for any penalty fees for cancellation, only a 30 day notice. This notice was served by 31 October 2013 but my account was still debited on 1 November 2013.
I phoned the Rivonia branch today where I was advised that according to the system, my contract will expire on 1 January 2014. Subsequently, I called the Head Office at the Wanderers where I was told differently by Customer Service that my cancellation was declined because they did not receive my documents.

I do not care for your lack of efficiency and administration issues within your company. I want my contract cancelled and the debit order removed with immediate effect. I have the required documents as proof of cancellation. This is not a request for your discretion, you are not authorised to debit my account. I will not hesitate to take legal action should you not rectify this mess before the next debit order is due.
