Claim for a lost phone

I bought a cell phone at game store at jabulani mall last year in April and requested for the phone to be insured immediately, i made payment at the same time and gave in my banking details for the money to be debited each month.i received a call from hollard insuance to confirm my banking details and all was sorted out.i lost my phone in february and called the insurance company for claim and only then i get told that my insurance was cancelled.
i did not authorise to cancell my insurance and the information i get was they sent me an sms stating the to notify me that that payment did not go through which i did not receive. i have never received anything to notify me that my insurance will be cancelled.
i spoke to the manager gohn or george and advised me to send my infomation through and a motivational letter with regards to the matter which i did and ever since i have not heard any feed back, i sent emails several times and im still not getting any feedback.
