Account application was amended to add charges

Applied for a foschini account in November 2012.There were sections that i skipped because i felt it was not important for me, and the sales lady advised that some columns i can just skip i.e debit orders, extra products etc.

To my surprise, the statement kept on increasing and when i checked my account on-line, i realised that there were charges of clubs, including some child clubs. Now why would i be charged for such when i don't even have a child.
I the requested foschini/TFG to cancel such and reverse the charges. They then responded by attaching my application form to prove that i did apply for those.

I was shocked to see how information was added onto my form. The retailer did not conform anything with me prior to implementing but now they do not want to agree that this is not the information provided by me. I marked with an X on the relevent fields, the amendments are made with ticks (whether somebody ticked to say i agree ar a cancell this is only known to them), i do not even know what the ticks suggest.

I want them to reverse the charges and amend the form accordinly becasue this is a total misrepresentation.
