Dstv not refunding annual subscription

I have an annual subscription to Dstv.

A lady from Dstv phoned me during December to remind me that my annual subscription is due. Paid annual subscription in December 2012, and then Dstv also debited my bank account with the same amount on 1 Jan 2013. Effectively billing my double for 2013. Mistakes happen, so I contacted them.

Phoned call centre. Was told billing staff only start on the 7th of Jan. I accepted that and was told they'll refund me.
Phoned end of Jan/beginning Feb. Was told they made a mistake sending the correct amount to billing, and that nobody followed up.
Phoned again on 8 Feb (was told will be fixed within three days) and 12 Feb (was told will be fixed within 7 to 14 days).
Today is the 25th of Feb. Got told the same story again. Incorrect amount sent to accounts. Promised 48 hour resolution.
I requested that supervisor phones me back, as I've spend enough time trying to follow up on my own. This has not happened.
