Debit order taken Despite cancellation

I have emailed my termination of subscription in December 2013!!! I'm beyond angry now.
I handed 1 calendar months written notice cancelling my subscription due to poor service (Failure to notify clients of system changes, cancelling signal without prior notice, removal of channels without notification, lack of response to calls etc etc) I received a call from one of the call centre representatives, apologising for their poor service and advising that my cancellation would be attended to. Mid January 2014 I received an sms indicating that my debit order was in the process to be run for the next months subscription, I immediately called again and spoke to another rep who advised she would remove my bank details to avoid them monies being debited. Lo and behold, 1 Feb 2014 the monies are taken from my account!!! I call the centre AGAIN and repeat the whole debaucle to yet another rep who advises that the accounts dept will contact me Monday (03.02.2014) to arrange a refund. Guess what, NO CALL! I call AGAIN this morning (Tuesday 04.02.2014) only to be told that I will need to go to the bank myself to get my money back! This is pathetic! No-one calls back and then I do your job!
