How does one obtain a Return of Earnings Form?

On 18 April I received an SMS informing me that our ROE was due on 30 April and inviting me to submit online by going to [URL Removed] I went to the website and registered, quickly receiving a username and password by e-mail. I have since been trying to download the form and on each occasion I receive a message "Your request for ROE access has failed... transaction rolled back". I have tried to phone on all the phone numbers supplied on the Compensation Fund letterhead but have yet to have the phone answered. In desperation I tried to fax them and... the fax lines were all engaged. I have now been trying for 6 days to contact them, simply to obtain a blank form to complete. Why would one need to be authorised to obtain this form?? When we fail to meet the deadline for submission due to the apparent impossibility of contacting the Commissioner by any means, we will no doubt be expected to pay penalties - an absurd situation.
