16&18 April - phone 0860 121 141 - Claim is with Assessor for Assessment
30 April - Xolile Sithole phone me to require a General Practitioner report
30 April - e-mailed the report to Xolile Sithole
7 May - phone them - Claim is with Assessor for Assessment. Need to follow up next Friday
20&23 May - phone them - Claim is with Assessor for Assessment.
27 May - phone them - Waiting for General Practitioner form. Was send to Dr Koekemoer by Xolile Sithole to complete. Xolile advise me that they do not have any timelines for claims. They can take as long as they want.
29 May - phone them - Waiting for General Practitioner form
29 May - Phone Dr Koekemoer must make an appointment to complete form. Was done.
30 May - Form was faxed through
12 June - Phone them - they received the fax
19 June - Phone to follow up - need the MRI report - was e-mailed to Elwin Mosale
2 July - Phone to follow up - need more forms to be completed
