Charge collection fee after invoice was settled

In 2012 I took my child for an appointment to an Occupational therapist and then my child's father and I went for a follow up visit.
It took more than 6 months and my various emails to her to send me the first invoice for R 1, 521.00 by the time I received it I was no longer in a financial position to settle the invoice properly. I went through a retrenchment process and lost also lost all my electronic documents from my place of employment during that time.
I believe in December Jenny called to ask why the bill hasn't been settled, I explained my financial situation and I think I spoke to her in Feb again to explain that our IT lost my software. She eventually re sent the invoice in August 2013 to my child's father - he immediately paid R 200 I then settled the account on the 1st November 2013 and requested for her to send the final report. TO DATE I have not received the report. I had also previously felt I was overcharged and but she refused to discuss.
My complaint through this whole mess is: 2 weeks AFTER I emailed the proof of payment she charged me a collection fee AFTER THE FACT. She even changed the invoice and backdated the charge??
