Bonitas unresolved issues pending already formonts

Nr. 47001371052. I applied for membership as from 1/9/ 2013 early in 8/2013. Membership only granted from 1/10/ 2013. I objected and received no further feedback despite several calls and e-mails. During middle November after several more phonecalls I received a membership certificate and membership card still showing 1 October 2013. I am not prepared to pay for the month of October as my membership as I have not received any proof of membership and also all my queries was ignored. After receiving my card and certificate in November, I paid two months installments for 11/2013 and 12/2013 and asked for joining date to be moved to 1 November because of the above. Futhermore I was charged late joiner penalty fees while me and my son was members of medical aids for all our lives. Bonitas application form requires last mem. cert. and does not state "all previous certificates"? I'm fedup with phoning in and not getting any response or problems sorted out.
