Refusing to pay authorized claim

Received authorization via e-mail and SMS on 24/01/2013 for a back procedure to be done in Umhlanga Hospital. Went for procedure 28/01/2013. Accounts were forward to Medical Aid and they declined the payment saying that they did not approve it even though I have the approval in writing from them. Doctor had to send additional correspondence to the Medical aid explaining all. As of today 14 March 2013, I am still waiting for the outcome.
