Incompetent admin service, now we have to pay

To whom it may concern,

PLEASE HELP ME.

Our doctor's office made a mistake on one of our claims, and now they expect us to pay.
We received a statement from the doctors with an outstanding amount (which was very strange, as we have a medical aid to cover all doctor's appointments), and they want to hand us over to their attorneys now. The problem is: the doctor wrote down the wrong name when we were there with the consultation. (Mistake 1 from the doctor's office). According to medical aids, even if you make a spelling mistake, they will still put the claim through. So if the lady in the office did her job like she had to, our medical aid would have paid for this visit. (Mistake nr 2 from their office). Now the lady says that we signed a form saying that we will still be held liable for all money outstanding on our account. The thing is, that disclosure said that if the medical aid did not pay for the visit, we still have to (but they never gave the medical aid the opportunity to pay for it). Now they just pull up their shoulders and say it's not their problem, we still have to pay for it.
Please help - we made no mistake, but have to pay for their mistake.
